I need a sample hypertension care plan. Can anyone help me with making one or finding a sample somewhere? -

I need to develop a care plan for a patient with hypertension and include teaching self measurement of B/P after discharge. Can anyone help me to write one or find a sample one. Thanks - I really need the help!

For taking blood pressure purchase them a digital BP cuff - you just slide it on your arm and push a button - it will inflate amp; deflate and display the results on a screen. Many of them will also save the results up to 10 tests. They also come in large display models for older people with sight impairments also.

IF ONE OF THOSE IS USED, A POTASSIUM SUPPLEMENT SHOULD ALSO BE USED AS IT IS DEPLETED BY INCREASED URINE FLOW.

The best one for your client/patient/family is the healthy heart diet, the trick is to lower the salt intake but unless the doctor prescribes it , there has to be certain amount of salt in diet to be healthy. Weight watchers have very good recipes as does any diebetic cook book.. learn to cook with spices..Mrs. Dash is a good alternative. Read all packaged food and that includes all bread and look at the sodium levels. Fish is one of the best foods to have (not fried) as it contains omega 3 oils, that help the bad fat... hope that gives you some ideas
