i have to do a project on dentistry and i require some case studies...so anyone who has undergone a major dental therapy pls pls pls fill out the questionaire below...as you would before your treatment.. AGE: SEX: HISTORY OF PRESENT ILLNESS: Narrative describing the patient s story of illness to include: chronology, bodily location, quality, quantity, setting, aggravating or alleviating factors, associated manifestations, past evaluations and/or treatment. PAST MEDICAL HISTORY: Childhood health; adult health ( surgery, history); accidents and injuries FAMILY HISTORY: Family health record: age, diagnoses, family disease diabetes, heart disease, hypertension, bleeding disorder. SOCIAL HISTORY:(SH) personal habits (sleep, exercise, diet, smoking, caffeine, alcohol and/or drug use). Review of Systems (ROS) :As recorded from most recent medical visit- mouth (toothache,gum bleeding,tooth sensitivity,cavity,plague,injury) Treatment advised and undergone:no of sittings,position of tooth,post surgical advices etc...
